Forms for Clients

There are certain forms that are necessary for new clients to complete at the beginning of therapy. They are useful for counselors to understand clients, for billing insurance, and/or for meeting legal requirements.

Please complete the Client Intake Form and the Consents Form before your appointment. When you arrive at your appointment, you will receive any other information or forms that are relevant.

If you have questions or wish to leave some sections of the forms blank, you may discuss this with your counselor.
